Our Sub-Acute Care

Pine Haven’s Rehabilitation program is professionally staffed and equipped to provide the best of a broad range of services. Our interdisciplinary treatment teams consist of physical, occupational and speech-language therapists, as well as a caring skilled nursing staff oriented to an individual’s needs. The goal is to assist each resident in reaching an optimal level of functioning in order to return home safely.

Whether the length of stay is several days or several weeks, a personalized plan of care assures appropriate medical, social and therapeutic services at every stage of the rehabilitation process. We also have a licensed dietician on staff to evaluate every resident’s dietary needs.

Additionally, all of our residents enjoy a wide range of amenities, including cable television, phone services, hairdressing services, group activities and religious programs.

Pine Haven’s Skilled Nursing Facility offers its residents the opportunity to live life to the fullest. Our registered nurses, licensed practical nurses, and certified nursing assistants have been specially chosen for their skills and caring attributes. Our physicians, nurses, therapists and support staff work together to provide individualized programs of care and rehabilitation. Their goal is to support and enrich each resident’s independence and quality of life. We are the caring, comfortable, comprehensive choice for people requiring skilled nursing care. It is our belief that each individual, no matter his or her age or infirmities, has the right to care that emphasizes dignity, respect and self-worth. Each resident deserves the opportunity to be as active, creative and self- fulfilled as possible.

Activities and Other Services

Our professional activities staff offers leisure time programs to suit individual interests. All activities are wheelchair accessible and planned for maximum involvement and variety. This allows for arts and crafts, discussion groups, singing, social events and parties -- all which encourage a sense of community.
