Case Management
Case Management is a service which is designed to make the service delivery system more
accessible and responsive to the individual needs of clients and to provide continuity of care
as client’s needs change. It serves as the focal point for identifying the needs of clients and
informal caregivers. Specifically, case management organizes service access for an individual.
It is the linkage among the client, informal caregivers and formal service providers which
facilitates an appropriate, comprehensive and coordinated response to the client’s needs.
The Office for the Aging Case Manager assigned to you serves a variety of functions depending
on your needs. These include:
- Screening
- Assessment of needs
- Comprehensive care planning
- Determination of eligibility
- Arranging and authorizing services
- Follow up on service delivery
- Client monitoring
- Reassessment
- Discharge planning
Some Office for the Aging services require that you accept Case Management. For others it
is a voluntary service based on the needs of the client and the availability of staff.
