HOURS: 9 a.m. – 5 p.m. (Monday – Friday)
Please allow sufficient time for processing of your passport. Normal
processing is six (6) weeks. If your departure time is less than
six (6) weeks, please see the attached information sheet for EXPEDITE
PROCESSING.

FEES:
• For all persons sixteen (16) years or older a ten (10) year passport
will be issued. The fee is $67.00 payable by check or money order
only (NO CASH) to the US Dept. of State*.
• For those applicants under sixteen (16) a five (5) year passport
is issued. The fee is $52.00 payable by check or money order only
(NO CASH) to the US Dept. of State*.
*PLEASE NOTE A $25.00 FEE IS ASSESED FOR EACH CHECK RETURNED, THIS
WILL ALSO DELAY THE PROCESSING OF YOUR APPLICATION!
• All
applicants: $30.00 fee, cash or check payable to the Columbia
County Clerk* returned check
*Please note the fee for returned checks to the Columbia County
Clerk is $20.00
PLEASE BRING THESE ITEMS TO OUR OFFICE ALONG WITH YOUR FEES:
1. Proof of Citizenship:
• Birth Certificate with a RAISED SEAL and filing date, certified by
official custodian of the record. NO HOSPITAL CERTIFICATES ARE ACCEPTED or,
• Previous US Passport, or
• Certificate of Naturalization
2. Identification:
•
Current driver’s license or valid passport
3. Two (2) passport photos (photos can be taken at the County
Clerk’s
office)
•
Black & white or color (white or light background)
•
2” x 2” in size; photos must be identical!
•
Head-size no smaller than 1” or no larger than 1 3/8”
A birth certificate is primary evidence of U.S. citizenship for
an applicant born in the United States. To be acceptable, a birth
certificate must show the following:
• Name of the child (including given name)
• Date and Place of birth
• File date (should be within one year of the date of birth)
•
Authorized seal of the registrar’s office (seal may be raised,
embossed, impressed or multicolored)
Every state, city or town now issues birth certificates with all
of the characteristics listed above. No exceptions are made for applicants.
Unacceptable evidence of citizenship:
• Hospital
Certificates
• Baptismal Certificates
• Notifications of birth
• U.S. Census records
• Any combination of the above
Please be aware that unacceptable birth certificates results
in the delay of the processing of your application due to correspondence
between the Passport Agency and the applicant.
